Smirnoff will not accept printed out rebate forms. The store I bought 8 bottles of smirnoff vodka did not have the rebate form. Where in AZ can I get an original rebate form?

Hi all. Well I jumped on this deal when I saw this post and just got back my rejection from the $60 rebate (i bought 8 bottles of Smirnoff). They are claiming that AZ was not a valid state for this specific rebate. I am usually pretty good about reading the fine print and I didn’t see this nor did I make a copy of the rebate form. Did anyone else have this issue? I can’t for the life of me find a copy online anywhere of this rebate to prove otherwise. So bummed.

I do thoroughly read ALL of the rebates & offers before posting. That’s why I post many deals well behind other sites. I make sure to get a picture of the deal, and all the details prior to making anyone run & spend money.
I no longer have the rebate, but it was ONLY valid in 2 states – CA and AZ. If it wouldn’t have been, I would not have posted the offer.
First off, Sheryl my post wasn’t to imply you posted anything false, you have saved my family & business so much money. I know how thorough you are & it’s appreciated. I was just trying to see if anyone else got rejected. The rejection card states they have kept all of my documents so I am going to request they send them back. Do you have any other suggestions on how to go about this? I have done these rebates in the past & have never had any issues.

rebate form for LA and WA only states and they rejected my submission. But

I guess it should be in their address. My form for LA and WA has this address where to send rebate: CMS Rebate Center, Attn: Save Up to $60 off Spirits, DIAGEO3933,
PO Box 426014, Del Rio, TX 78842-6014. I believe for CA they have something
different in this address or DIAGEO####. Could someone who was so lucky to have
the correct form check it for us please?
Hi Jessica, this is exactly what happened to me. The form that I had was for LA/WA only. I just spoke with the CSR and she gave me the same advice so hopefully someone did in fact do the rebate in CA or AZ and will just respond with the offer code. And the code would also be printed on the rebate check as well. I guess we will just have to keep our fingers crossed.
